Sanctuary Personnel is currently seeking an experienced Band 6/7 Bed Discharge Coordinator (RMN) to take on a new locum opportunity based in Greater Manchester.
Location: Greater Manchester. Completing paperwork etc can be either WFH or office based depending on preference.
Contract Type: Locum, 3-month contract
Pay Rate: £30 - £31 per hour (assignment rate)
Duties:
- Assessment work would require visiting patients on the ward
- Unblock barriers to discharge for patients in our contracted beds within the North West (mainly at Cheadle) and the local spot purchase beds
- Supporting care coordinators to complete assessment work and taking cases to panel or working with patients who require allocation of a care coordinator
- Working with a team of other qualified practitioners who over see all the patients in out of area placements but this role is exclusively to complete the function of a hospital discharge worker/ care coordinator
Requirements of the Bed Discharge Coordinator:
- A qualified nurse or social worker at band 6 or 7 level who has experience of working in Manchester
- Relevant registration
- Essential to have care coordinator experience and have previously completed social care assessments, put together costings and budgets, experience of taking cases to Manchester social care and/ or ICB panel and placement finding
